• Step 4 - In Option Section --> Type your question option that you want to create for that question
• Step 5 - If you want More than 4 Options then select Add Options Button
• Step 6 - If you want give explanation for the questio then Type your explanation in that Explanation Box
• Step 7 - If you want add another question please select Add Question Button
• Step 8 - Repeat above Step 1 to Step 6
